Ms. Jenna Dickman had given birth to triplets in your hospital on May 1st, 2012. Her Father was there, a Mr. Ron Dickman. We are having custody issues at this time in Los Angeles. He recently produced a "Father On Record" form, that has HIS name on the form. He is Jenna's Father, not her childs Fathers. Besides the obvious reason, (Incest), this form must bee corrected immediately. I know I am the Father, just by resemblance, but will obtain a swab. I need this record, or any and all records that would denote: 1) Certificate of Birth 2) Copy of Father of Record certificate, or other simular affidavits.

My name is GARTH POWELSON, and for the life of me, I do not see how this form was made, or obtained, without proper testing such as swab, blood, etc. Thank You For Your Response- Garth Egan Powelson (

The psych floor is very good. The employees are very nice, treat the patients well and easily communicate with the family of the patient. They have information groups that include families and patients together to disseminate information. That was very helpful as patients are able to respond to questions and inquiries. However, based on insurance, they are quick to exit patients, sometimes while still sick. The door revolves too quickly.
